Abstract   Efficacy of recombinant humsn G-CSF(granulocyte colony-stimulating factor: Neutrogin) was evaluated in 30 advanced lung carcinoma patients at Severance Hospital and. Yonsei Cancer Center during the period of August, 1992 to January, 1993. Every patient received same chemotherapy regimen at least 2 courses. Any patient who has the serious medical illness other than lung cancer or hypersensitivity to G-CSF was excluded. The first chemotherapy course without G-CSF was a control period for evaluation of the degree of neutropenia and the second course with the G-CSF was a evaluation period of G-CSF efficacy on neutropenia. G- CSF was administered subcutaneously with the dose of 2 ug/kg/day for 14 days. The efficacy of G-CSF was evaluated according to the criteria of Japan Cancer Treatment Society. The following results were obtained. 1) Total number of enrolled patients were 30: twelve(40.0%) with the small cell lung cancer and eighteen(60.0%) with the non-small cell lung cancer. The chemotherapeutic regimens were EAP, VIP, MIF and CA V. 2) Mean nadir of WBC counts during the control period and evaluation period were l334.0 +- 689.2/mm(3) and 3259.3+- 2190.3/mm(3), respectively(P < 0.001). 3) Mean nadir of neutrophil counts during the control period and evaluation period were 311.3+- 263.2/mm(3) and 1915.1+- 1989.0/mm(3), respectively(P < 0.001). 4) Mean duration of WBC counts less than 1000/mm were 1.7 +- 2.67 days in the control period and 0.6+1.89 days in the evaluation period(F<0.02). 5) Mean duration of neutrophil counts less than 500/mm were 4.9+-3.0l days in the control periad and 0.7 +- 1.69 days in the evaluation period(P < 0.001). 6) The recovery time of WBC from nadir to 3000(3) was 7.3+-4.4 days in the control period and 1.9+-1.94 days in the evaluation period(P<0.001). 7) The recovery time of neutrophil from nadir to 21500/mm was 6.7+-3.73 days in the control period and 1.5+-1.57 days in the evaluation period(P<0.001). 8) Side effects of G-CSF were mild back pain observed in four patients(13.3%), mild fever and general weakness in one(3.3%) respectively. Slight elevation of transaminase was observed in one case(3.3%) which was normalized before next chemotherapy cycle without specific treat- ment. In conclusion, G-CSF was very useful in 23 case(76.6%) and useful in 7 case(23.4%) for chemotherapy induced neutropenia in the lung cancer patients. The side effects of G-CSF were acceptable.
